Dear Tournament Player/Parent,

Please HELP US GO GREEN. Save Trees and time and UPLOAD your medical waiver to us using the Match Tennis App. After registering for Match Tennis App it will auto-fill your personal info and then you simply sign the waiver with your mouse or finger and click the “upload” button. If needed you can print your waiver out also. We will have a limited number of waivers on site.

Medical Waiver Upload Directions:

1. After you register for Match Tennis App use the Match Tennis App Medical Waiver tool to fill out and UPLOAD your waiver directly to the tournament director.
2. Upon completing the form click the “Create PDF & Upload” button….
3. Please make sure you see the “Medical Waiver Uploaded” notification popup for verification your waiver was uploaded. If not repeat and make sure all form fields are filled out properly.
